2018 WOSL Team Membership Application

All existing Teams must fill out a Team Membership Application Prior to our December 10, 2017 Annual General Meeting.

This year the Application Form is an online form.

All Fees must be handed in at the Annual General Meeting.

All Fines must be paid by November 30, 2017 before a Team Membership Application will be accepted.

Existing Teams submitting Team Membership Applications after the Annual General Meeting will only be accepted at the discretion of the Executive Council and may be subject to a $200 late fee and possible placement in the lowest division.
